LOOSE Saggy SKIN

When we lose weight, age, or spend too much time in the sun, our skin can become stretched and saggy.

The Science

The most common causes of loose, sagging skin are:

  • Aging: As we age, the production of collagen and elastin in the skin decreases, leading to a loss of firmness and elasticity
  • Genetics: Genetic factors can influence how quickly your skin loses elasticity.
  • Pregnancy: The skin stretches significantly during pregnancy, and it might not always return to its pre-pregnancy state
  • Rapid Weight Loss: Quick weight loss can result in excess skin that has not had enough time to shrink back
  • Sun Damage: Prolonged exposure to the sun’s UV rays can break down collagen and elastin fibres in the skin

Strategies to improve loose, saggy skin includes the following oral supplements:

  • Vitamin C Supplements: Collagen contains an amino acid called proline. For our body to produce collagen, proline needs to be converted into another compound called hydroxyproline, and Vitamin C is required for this conversion to occur. Consuming vitamin C; at 500mg twice daily helps increase our body’s own production of collagen.
  • Collagen Supplements: Collagen is a protein found in various parts of our body, including bones, tendons, skin, and muscles. Collagen supplements contain certain amino acids, notably hydroxyproline, glycine, and proline. When ingested, these supplements are broken down into smaller pieces that are absorbed into the bloodstream. Collagen supplements are derived from animal sources making them unsuitable for vegans and vegetarians.

A diet rich of foods rich in collagen and Vitamin C can support skin health and improve loose saggy skin:

  • Collagen-Rich Foods: Beef, bone broth, chicken, eggs, fish, gelatin, pork, shellfish, turkey, wild game.
  • Vitamin C-Rich Foods: Acerola cherries, bell peppers, blackcurrants, broccoli, Brussels sprouts, cantaloupe, cauliflower, citrus fruits, guavas, kiwi, kale, mango, papayas, parsley, pineapple, potatoes, raspberries, spinach, strawberries, tomatoes, watermelon.

While the effectiveness of facial exercises is debated, some studies suggest that targeted exercises might improve muscle tone and volume in the face. Additionally, facial massage tools can stimulate fibroblasts to produce more collagen, though results are often minimal.

Devices used to firm and tighten loose skin use heat to stimulate collagen production and tighten the skin. The collagen molecule is like a triple helix held together by crosslinks. The process involves heating the collagen fibers. When collagen fibrils—the small fibers of collagen—are heated, they undergo changes. Firstly, the collagen denatures when it has been heated to 65°C. This means that the crosslinks break, making the collagen contract even more, resulting in a shorter and thicker form. This immediate contraction of collagen means tissue contraction, which reduces the skin’s looseness and gives it a lifted look. In addition, the heat and mechanical force of the device damage the skin in a controlled manner, which then triggers the body’s natural healing process, leading to the formation of new collagen by active fibroblasts. Interestingly, the time needed for collagen to denature and contract depends on the temperature: the hotter, the faster.

The appearance of loose, saggy skin is characterized by the skin’s lack of firmness and elasticity. This condition can affect various parts of the body, including the face, neck, arms, and abdomen. Clinical features include:

  • Loose, hanging skin
  • Fine lines and wrinkles
  • Reduced skin thickness
  • Dull skin texture

These symptoms can be distressing and impact one’s self-esteem and confidence. Understanding the underlying causes and available treatments is crucial for managing this condition effectively.

Loose, saggy skin is a common concern that can arise from various factors such as aging, genetics, pregnancy, rapid weight loss, and sun damage. Skin tightening creams, serums, and in-clinic treatments can provide temporary and sometimes long-lasting results. However, maintaining a consistent skincare routine, protecting the skin from sun damage, and supporting skin health through diet and supplements are essential strategies for managing and preventing loose skin. While younger skin generally has a higher concentration of elastin and may bounce back more quickly, older skin or skin that has been excessively stretched may not fully return to its original state. In such cases, surgical removal might be considered.

Solutions

Devices used to firm and tighten loose skin use heat to stimulate collagen production and tighten the skin. At the Marr Clinic, the available treatments are:

Other Treatments and Strategies

A treatment that is not available at Marr Clinic that can address loose, saggy skin includes radiofrequency (RF) treatments, like Exilis. Exilis uses radiofrequency energy to heat the deeper layers of the skin. This heat stimulates collagen production and tightens existing collagen fibers, resulting in firmer skin. RF treatments are non-invasive and suitable for various areas of the body. This strategy together with the strategies of using oral supplements and consuming a diet of foods rich in collagen (as detailed ‘The Science’ section will improve loose saggy skin.
